Default Uneven tire wear
I heard that uneven wear is due mainly to improper inflation, however, I am pretty religious about tires.

My front tire has 12,000 miles on it and the left tread is worn more than the right. Any theories? Is this a safety issue?

It can be from taking more left turns than right, ie. freeway on ramps, or traffic circles. And it can cause the bike to handle a little funny, 12000 miles is a lot on a sport bike tire so I'd replace it.
