Bad news Jason. Main thing is you aren't hurt.
So it looks like a panic grab of the brakes lost the front and she went down on the right. Then slid nose first into the bumper causing the main damage on left headlight area.
Thats a shame because if it didn't go down, you may have been far enough back to stop in time
Like Trickster said, we've all had close ones in the past, you were just unlucky not to get away with it.
I hope the insurance works out and they do a good job. If any panels are lightly damaged, insist on replacement parts instead of repair and paint, because if you are putting in a claim, you might as well do it properly.
